Telehealth Consent & Disclaimer

This Telehealth Consent and Disclaimer (“Consent”) describes how Desert Sage Primary Care (“Practice,” “we,” “us”) provides healthcare services via telehealth to patients located in Arizona. By utilizing our telehealth services, you acknowledge and agree to the terms below.

1. Definition of Telehealth

Telehealth involves the use of electronic communications (video, audio, and asynchronous messaging) to enable healthcare providers at different locations to share individual patient medical data for the purpose of improving patient care. The information may be used for diagnosis, therapy, follow-up, and/or education.

2. Expected Benefits & Risks

  • Benefits
    Improved access to care while remaining at home or work.
    Increased convenience and continuity of care.
    Faster follow-up for appropriate medical concerns.
  • Potential Risks
    Technical interruptions or equipment failure.
    Delays caused by internet connectivity.
    Incomplete information compared with an in-person examination.
    Although we use secure systems, no electronic transmission is completely risk-free.

3. Eligibility & Arizona Licensure

Desert Sage Primary Care providers are licensed to practice medicine in the State of Arizona. You must be physically located within the State of Arizona at the time of your telehealth visit. We reserve the right to verify your location prior to beginning any session.

4. Emergency Situations

TELEHEALTH SHOULD NOT BE USED FOR EMERGENCY CARE. If you believe you are experiencing a medical or psychiatric emergency, you should immediately call 911 or go to the nearest emergency room. Desert Sage Primary Care does not provide emergency intervention services via telehealth.

5. Privacy & Security

We utilize secure, HIPAA-compliant platforms for all virtual visits. However, it is your responsibility to ensure you are in a private location with a secure internet connection to protect your own health information during the call. Recordings of telehealth sessions are not permitted by either party without prior written consent.

6. Financial Responsibility

You are responsible for payment of all fees associated with telehealth services, including co-pays, deductibles, or self-pay rates as established by our current service agreement. Please consult your insurance provider to confirm coverage for virtual visits.

7. Consent to Treatment

By scheduling or participating in a telehealth session with Desert Sage Primary Care, you certify that you have read, understand, and agree to these terms. You voluntarily consent to receive medical care via telehealth technologies and understand you have the right to withdraw this consent at any time.
